Clinical Care Services

CarePlus provides in-service programs for senior living staff led by experienced RNs. With more than 30 years of experience caring for older adults, our nurses share practical information that helps staff recognize changes in residents, communicate concerns, and understand when additional care may be needed.

CarePlus offers educational workshops and wellness programs for staff, residents, and families. Programs focus on common health concerns facing older adults and provide useful information that can be put into practice. Topics may include:

  • Lunch & Learns on current health and aging topics
  • Fall prevention
  • Blood pressure checks and monitoring
  • Healthy aging and wellness
  • Infection control
  • Medication awareness
  • Chronic disease education

CarePlus provides hands-on training and support for senior living staff. Our RNs work directly with staff to address resident health concerns, changes in condition, medication needs, and situations that may require additional clinical attention.

CarePlus works closely with care managers, physicians, and medical practices to help ensure care plans are followed and everyone involved in a resident’s care stays informed. Our nurses provide updates, coordinate care, and communicate changes or concerns as they arise.

CarePlus works with hospitals and discharge planners to help older adults return safely to their home or senior living community following a hospitalization or rehabilitation stay. We can coordinate services before discharge and provide nursing care, medication management, and personal care once the individual returns home.

A stroke, hip fracture, surgery, hospitalization, or other change in health can quickly change a resident’s care needs. CarePlus can provide additional nursing or personal care during recovery or when a resident requires a different level of care.

We work closely with the resident, family, community staff, and healthcare providers to keep everyone informed and ensure continuity of care.

CarePlus brings more than 30 years of experience caring for older adults to our education and training programs. Our RNs provide presentations for staff, residents, and families on healthy aging, chronic and complex conditions, medication reminders, care transitions, and other topics important to older adults.

Programs are developed around the specific needs and interests of each community.
