24-hour care may be needed after an emergency situation when a person has recently been hospitalized or has stayed in a rehabilitation facility after a bone fracture, stroke, or heart attack. When changes come more gradually, there are signs that can help family and friends recognize that an older adult needs 24 hour care.

A common need for 24/7 care comes when family or friends are concerned about leaving an older person alone at night. CarePlus can help. A professional home caregiver can change all of this. All are trained to assist with Activities of Daily Living (see above). Additionally, there are memory care trained caregivers who focus on patients with changing cognition.
Our caregivers are trained to calm older people who may become agitated because they have lost their sense of time and place, help avoid kitchen or bathing accidents, help prevent dehydration, fainting and falls, as well as implement best practices to reduce infections through a 24-hour care regimen.
Caregivers provide:
A routine and schedule for our clients that is both familiar and comfortable;
Help with Activities of Daily Living by running errands, shopping for groceries, tidying up the house, doing the laundry and taking the client to doctor appointments
Assistance with Personal Hygiene/Self-Care: bathing/hair washing, dressing, toileting and grooming
Additional Support for: medication reminders, meal prep and exercise
Transitional care while waiting for placement in a senior living community

The CarePlus skilled nursing team includes a Registered Nurse, a Licensed Practical Nurse, a Certified Nursing Assistant, and a Geriatric Nursing Assistant plus specialists in particular diseases: COPD, pneumonia and other respiratory conditions, cardiac conditions, diabetes, Alzheimer’s Disease, Amyotrophic Lateral Sclerosis (ALS), Multiple Sclerosis, and Parkinson’s Disease.
We will work with your family to determine what type of skilled care your family member requires.
An RN or LPN can respond to rapidly changing health conditions and can perform complex wound dressings, rehabilitation services, tube feedings, and other highly skilled procedures.

CarePlus can provide support with follow up visits to doctors, new medication regimens and develop a phone log for emergencies.
Our 72 Hour Plus Discharge Program provides a personal caregiver for up to 6 hours each day for the first 3 days post-surgery with nursing (RN) oversight by providing:
